Grants to Kentucky nonprofit and faith-based organizations, and government agencies in eligible communities to promote the health and well-being of local residents. Funding is intended for programs that support the four components of well-being: purpose, health, belonging, and security. The program addresses the social determinants of health by funding initiatives that provide critical safety net services and those that make Louisville a more appealing place to live for all.

In an effort to encourage collaboration among nonprofits, grassroots organizations and a multitude of other sectors, The Humana Foundation intends to fund as many collaborative efforts as possible, with joint applications from two or more organizations.
