Grants to Kentucky nonprofit and faith-based organizations, and government agencies in eligible communities to promote the health and well-being of local residents. Funding is intended for programs that support the four components of well-being: purpose, health, belonging, and security. The program addresses the social determinants of health by funding initiatives that provide critical safety net services and those that make Louisville a more appealing place to live for all.

In an effort to encourage collaboration among nonprofits, grassroots organizations and a multitude of other sectors, The Humana Foundation intends to fund as many collaborative efforts as possible, with joint applications from two or more organizations.

Additional Eligibility Criteria:

Tax exempt organizations, including nonprofits, governmental organizations, and faith-based organizations, are eligible to apply.

If your tax status is anything other than 501(c)3, a fiscal sponsor is required to be eligible to apply. You will be able to designate your fiscal sponsor status within the online application.

The Humana Foundation does not provide funding for sponsorships and fundraising events.
